This Agreement is entered into as of the date By and between Party Name (“Seller”) and Party Name (“Buyer”).

WHEREAS, Seller and Buyer entered into Purchase Agreement dated date (The “Original Agreement”), whereby Buyer agreed to purchase and Seller agreed to sell property located at address (The “Property”); and
